Completing and submitting a claim online:

Once you begin the online submission process, you’ll receive an email with a code to validate your information and complete the claim form. Note: the City will not be able to take action until a complete claim form is submitted.

Complete a claim form by mail (or return in person):

Download and complete the claim form. Mail or bring the signed original form with wet signature to:

  • City Hall is open to the public Mondays, Tuesdays and Thursdays, 8 a.m. – 5 p.m.
Please submit your completed signed form along with any accompanying documents.  Missing information may delay the processing of your claim.
 

Processing

All claims received are forwarded to Sedgwick, our third party administrator. Sedgwick investigates all claims and makes recommendations. The Sedgwick claim examiner will contact you, as the claimant, to discuss the claim and determine damages. The claim examiner then notifies us as to whether the claim should be accepted or denied.

Note: In accordance with California Government Code, claims resulting from damage to personal property, personal injury or death must be submitted within six months of the loss, injury or accident. Claims concerning real property must be submitted within 12 months of the loss.
