Important Notes:
• Knowingly submitting a false Crime Report is a misdemeanor under Section 148.5 of the California Penal Code and is punishable by up to a year in jail.

Instructions:
• This online form should only be used for the crimes indicated in the list appearing after these instructions. If it doesn't fit into one of these categories, then you should report the crime in person at the San Carlos Police Department, 600 Elm Street, San Carlos or by telephone (650) 802-4277.
• To report violent crimes such as assault, kidnapping, robbery, etc. call 9-1-1.
• To report stolen license plates or vehicles including trucks, trailers, etc. call (650) 802-4277.
• To report traffic accidents involving injury please call 9-1-1.
• Please use this form only to report crimes that occurred in the City of San Carlos.
• Please complete as many lines of the report as you possibly can. This will help us investigate the crime that you are reporting and /or screen it for potential investigation.
• Whenever you only have partial information (like part of a license plate number, or a person's description), please give us whatever you have.
• If you need to report more than one type of crime (or multiple instances), please send us a separate report form for each one.

Stolen Property:
There are five pieces of information that are required when describing stolen or lost property. Enter the information in the labeled columns. If you do not know the information for a category, enter "Unknown". Please make every effort to locate and enter a serial number when applicable for lost and stolen items.

Bicycles: Indicate whether boys or girls, speeds, wheel size and color, serial or bicycle license number.
Cameras: Indicate size of the lens, flash attachments, description of accessories, etc
Cell Phones/Pagers: Include the serial numbers and phone numbers of each (Use the additional comments field to list the phone number.)
Computers: List each component separately, making sure to include all serial and model numbers for each item.
Jewelry: Indicate a complete description including type of metal (e.g.: gold, silver, etc.), size, type, number of stones, etc.
Radios: Indicate whether AM/FM, CD/Tape player, Walkie-Talkie, and additional features.
Stereo Equip: List all components, speakers, headphones and type of cabinets or serial numbers.
Televisions: Indicate whether color or black & white, screen size, console/portable, etc.

Note: This electronic crime report system is not a fast response system. You will receive a report number in the mail within 7 days from the time that you file this report
