Application to the City of San Carlos


On January 23,2004, the Palo Alto Medical Foundation (PAMF) submitted a formal Application to the City of San Carlos for the development of the PAMF San Carlos Center, a medical campus at 301 Industrial Road. Subsequent Supplemental Information and various clarifying correspondence are incorporated into this Document. The Application was deemed complete on August 31,2005.

This Application addresses the areas required by the City and provides an extensivedescription of the development, along with the requisite application forms and processing fees. The Application includes background information and details on community needs and benefits, business rationale, project description, architectural design and program concepts, site plan, circulation, transportation, traffic, parking, landscaping and development amenities.

This Application is the basis for securing the required entitlements to develop the proposed $300 million medical office and hospital project. The current use and zoning of 301 Industrial Road is M-1 (Light Industrial). The Application proposes re-zoning the property to PC (Planned Community) and approval of a related Use Permit. The Application also proposes changes in the General Plan to accommodate the proposed medical facility and includes a request for a Development Agreement.

The purpose of this Application is to obtain these necessary plan amendments and entitlements, in order to permit the development of the proposed PAMF San CarlosCenter. PAMF is requesting that the General Plan Amendment, PC Zoning Ordinance, Use Permit and Development Agreement all be processed concurrently.
