INSTRUCTIONS:  This monthly report is to be completed by the probationary officer’s supervisor and submitted to the Patrol Commander via the FTO Sergeant.  The officer is to be rated for performance during the period using the standardized guidelines on the rear of the form.  Specific comments are recommended for all ratings of 3 or less and 6 or more.  The evaluator is also encouraged to comment on any areas of superior or substandard performance.  Attach copies of officer-involved accidents and any unacceptable or superior reports by the officer.
